Self-Draw Blood Collection Device: This innovative device uses a gentle vacuum and microneedle technology for a less painful, easy collection from your upper arm capillaries.
Collection Supplies: Instructions, alcohol prep wipe, warming pad, collection tube, and a band-aid.
KlothoYears Sample Form: Important! Please complete this form with your information and include it with your sample to avoid delays.
Shipping Materials: Shipping envelope, plastic bag for your sample tube, a refrigerant gel pack (freeze before use!), and a prepaid FedEx 2-day air shipping label.
Shipping Your Sample to the Lab:
When to Collect & Ship: Samples must be drawn on a Monday, Tuesday, or Wednesday and shipped on the same day they are drawn.
FedEx Drop-Off: A prepaid FedEx 2-day service label is included. You are responsible for dropping off your sample at a FedEx location before their daily cut-off time which is usually 5:00 PM, but it may be earlier in remote areas.

For customers who send samples not drawn by our mobile phlebotomy service:
- Shipment of blood samples requires four levels of packaging:
- Serum or Plasma in a glass or plastic tube.
- Absorbent packaging (materials used to protect primary receptacles and absorb liquid if leaks.
- An insulated foam container with ice, refrigerant packs, or an integrated refrigerant device.
